We welcome re-enrollments from all PUSD schools and new/open-enrollment students from outside the Payson School District and other Arizona school districts.

IMPORTANT!  Online Registration is NOT compatible with mobile devices (i.e. SmartPhones, tablets). Please use a computer, laptop or visit one of our campuses to complete your student's annual update or new enrollment.

Enrollment Requirements

The following information will be needed to enroll your student:

• A certified copy of the pupil’s birth certificate; or other reliable proof of the pupil's identity and age AND a notarized Affidavit of Birth explaining the inability to provide a copy of the birth certificate (A.R.S. §15-828)
• Current immunization records
• Proof of residency (Arizona Department of Education Arizona Residency Guidelines and Affidavit of Shared Residence Form)
• Parent/Guardian state-issued Photo ID (or equivalent legal form of I.D. per Governing Board Policy JFAA)
• Withdrawal Form (If previously enrolled in an Arizona school)
• Guardianship/Custody Documentation (if applicable)

Have a change in your student(s) information? Print out the PUSD Student Information Change Form. This form can then be emailed directly to central.registrar@pusd.org (proper documentation must be attached for change) or taken to the Central Registrar at the PUSD District Office -  902 W. Main Street, Payson, AZ.
